Primary Lymphoma of the Ileum Manifested as Ileocolic Intussusception


Abstract
Herein, we report on a case of primary lymphoma of the ileum manifested as ileocolic intussusception.
A 34-year old male patient suffering from intermittentabdominal pain and vomiting for 3 months underwent a small bowel series and barium enema and ws found to have intussusception of the ileocdic type.
The intussusception was found to be caused by malignant lymphoma of the terminal ileum. Right hemicolectomy with lymph nodes dissection and ileo-transverse colostomy with a 31mm Valtrac-Bar(Gap 1.5mm) were performed.
4.1¡¿3.0¡¿2.0cm sized polypoid tumor with mucosal ulceration was present at the terminal ileum, 3cm proximal to the ileocecal valve. The cut surface was grayish, solid homogenous, rubbery firm, and slightly myxoid.
The pathologic type was malignant lymphoma with diffuse large cell immunoblastic plasmacytoid(NCI Working Formulation), diffuse histiocytic type(Rappaport) and the lymphoma cells extended to the outer muscle layer. The resection margins were
clear.
There was no metastatic growth in 38 regional lymph nodes.
At the 11th postoperative day, he received combined and alternative chemotherapy with "COPBLAM V" regimen (Cyclophosphamide, Adriamycin, Vincristine, Procabamazine, Bleomycin, Prednisolone).
He is in good health without any evidence of recurrence.
